Is your meat thermometer accurate?

You can easily check. Next time you bring water to boil, dip your meat thermometer in and let it register. It should be 212°F. If it's not you may be able to adjust it; check to see if there's a screw in front of the gauge housing. If so, use a pair of pliers to turn it and adjust up or down.
